Finding EXTRAordinary in the Ordinary

Victoria McLemore, PA-C is a practicing emergency medicine APC at Grandview Medical Center in Birmingham, AL.

Reason for Visit:
66-year-old male presenting to the ED with urinary retention after a recent bladder biopsy

Patient Outcome:
The patient developed septic shock with bacteremia. Because Victoria recognized the severity of his condition early and initiated the sepsis protocol from triage, fluids and antibiotics were administered immediately, before the patient was placed in an ED room. The patient was admitted to the ICU, received timely treatment, and was discharged in stable condition after five days.

EXTRAordinary Care

As the Provider in Triage, Victoria McLemore quickly identified that the patient appeared significantly sicker than the presenting complaint suggested. Despite no available rooms in the ED, she initiated the hospital’s sepsis protocol, enabling the nursing team to start critical interventions promptly, including fluids and antibiotics, before the patient was formally evaluated by the attending physician.

The patient ultimately developed septic shock and was diagnosed with bacteremia, requiring ICU admission. However, thanks to Victoria’s rapid assessment and decisive action, he had already received early, life-saving treatment well before the severity of his condition was fully apparent in the ED workup.

Her clinical judgment and proactive care directly contributed to the patient’s positive outcome, illustrating how timely action and expertise in triage can make all the difference when every minute counts.

“Victoria did an outstanding job quickly recognizing and treating this septic patient. Her swift action demonstrates the critical role our APC team plays in patient care, especially as providers in triage, where their expertise allows serious conditions to be identified and managed early. Since joining us as a new graduate, it has been incredible to watch her growth and confidence as a clinician. We’re proud to have her on our team.”
